Try this

One way I debug access issues for web is to enable domlog and see what id is actually trying to access the db.

My guess is

CN=Sally May/OU=TIA_Users/OU=ABCCompany/DC=trustco/DC=com should be added to the ltpa username field as

CN=Sally May,OU=TIA_Users,OU=ABCCompany,DC=trustco,DC=com

Domlog will tell you for sure.
